A Health Care Power of Attorney is a written document granting another person the power to make medical treatment and life support decisions for you when you are no longer able to make those decisions on your own.  This person is called your “Patient Advocate.”

Selecting an appropriate person to be your Patient Advocate is critical.  What could be more important than how you are treated by medical personnel when you cannot participate in that treatment decision?
